ADMISSION PROCESS

Reservations for short term or long term care are usually made in advance by the patient's physician, family, or hospital social service department. Inquiries and visits are encouraged. Rates are commensurate with the type of care needed and accommodations desired. The cost for care at our nursing home is considerably less than comparable nursing care in an acute hospital or for licensed in home care.
We are Medicare Certified and State Licensed. We accept Medicare, Medi Cal, most HMOs, Private Insurance, and Private Pay.
Our social services director will be able to make you or your family member's transition into our facility as comfortable as possible. Please come by for a tour or call us to discuss your options with our administrator or admissions coordinator.

*If coming from Santa Barbara Cottage Hospital or Goleta Valley Cottage Hospital:
Inform your discharge planner and physician that you would like to be discharged to Santa Barbara Convalescent Hospital. We will collect the necessary paperwork from your discharge planner and we will arrange a pick-up time for you to be brought to our facility.
*If coming from assisted living, board & care, or home:
Prior to admission, please have your primary physician fax medical information to (805) 687-1307. Information should include: Face sheet, insurance information, history & physical, current medications, as well as x-ray or lab reports.
*If coming from out of town:
Please arrange for a local physician to be the attending physician so they can manage your care while at our Santa Barbara nursing care center.
